I'm glad I found this. My initial Second Approver - which is sent upon e-form submission - doesn't send. My Second Approver 'reminder' emails go out eight (8) times each. On the bright side...we get their attention. As a result, I have mixed emotions about this getting fixed. I'm in 4.0.6.
